If you would like to request a copy of a report by e-mail, and are an authorized party, please complete this form.

Date of the Incident
Files must be less than 4 MB.
Allowed file types: gif jpg jpeg png.

Your request will be reviewed by our Records staff during normal business hours. Reports cannot be e-mailed back to you, however, we will contact you at the phone number or e-mail address provided to advise when the report is ready for pick-up.
